May heater blower motor died

This morning my heater blower motor stopped working. It had been making noise for the last month or so at the lower speeds, but it stopped completely on my way to work. It didn't just die, but it ran intermittently for about 15 minutes before it did. Anyway, I would like to know if anyone has had this problem and what all it involves to fix it. There are only two things it could be. It's either the motor itself or the fan switch. I am leaning more toward the blower motor since it had been making chirping noises.
